Annual Parental Consent Form

Any student taking part in a school trip must complete a Consent Form with current and up-to-date information.

This Form below is designed to cover standard, low-risk, non-residential and non-overseas visits.  

Please complete and submit this form at least two working days before the day of the trip your child is expecting to attend (paper copies are available from the School Reception).  If this is not completed and submitted by the relevant deadline your child will not be able to attend the trip.

Once completed, we then ask that this form is updated on an annual basis to ensure we have the most up-to-date information.

Residential/overseas/adventurous activities trips require an extended Consent Form. These forms will be issued by the trip organiser prior to each event.
