Please complete the form below to apply to Cannock Chase High School Sixth Form for a September 2018 start.
Under each subject menu, please select from the multiple choice options your most recent predicted grade.
The Level 2 pathway is a 12 month programme specifically for 16-19 year olds who wish to continue studying and who meet the entry criteria of 3 A*-D/9-4 qualifications achieved at Key Stage 4 (Yr11).
Students pursuing the Level 2 pathway will receive 10 hours of teaching in the pathway subject of their choice and will achieve a Level 2 Cambridge Technical Diploma after 12 months of study. Students may then wish to apply to study Level 3 programmes as part of the Sixth Form.
Students pursuing the Level 2 pathway will also undertake work experience, enrichment and resit GCSE English and/or Maths where appropriate.
The Level 3 pathway is the traditional programme of study in Sixth Forms: a two year programme specifically for 16-19 year olds who wish to continue studying and who meet the entry criteria for A level and BTEC Level 3 programmes (please see the 'Entry Criteria' link that can be found on the Sixth Form page of the website).
Students studying this pathway will typically achieve the equivalent of 3 A levels in order to progress to university, apprenticeships or employment post Sixth Form.
Please select from the option menus below, remembering that we encourage students to choose four subjects in Year 12 and continue with three in Year 13.
Please outline in this box any further information you wish to communicate to the Sixth Form Team.
You will know that you have correctly submitted the form as you will be directed to a new page that will state 'Thank you for submitting your application'.
